How to Apply for Community Grants
2017 – 2018 Request for Application
Applications will be accepted for breast health or breast cancer navigation, screening, treatment, or support projects for the medically underserved in the Chicagoland Area until November 28, 2016 for the fiscal year April 1, 2017 to March 31, 2018. Projects that specifically address the objectives outlined in the “Funding Priorities” in the application will be given priority.
The following RFA workshops were hosted by the affiliate:
- Cook County Tuesday, October 11, 2016 9:00am to 11:00 am
- Crystal Lake Wednesday, October 12, 2016 9:00am to 11:00am
- Virtual Webinar Monday, October 17, 2016 1:00pm to 3:00pm
At this time we will accept applications from non attendees, however, limited technical assistance will be provided. Click here for the PowerPoint Presentation.
Applications must be submitted via the GeMS system by November 28th, 2016 at 5:00 PM CST.
